E-cigarette use has been increasing worldwide among both adults and adolescents. There are several issues related to e-cigarettes. One important issue is relative harmfulness of e-cigarettes. E-cigarettes are not-combustible, so it is less hazardous than cigarettes. However, because e-cigarettes contain several carcinogens and nicotine, it is not without health hazard. Second issue is related to smoking cessation effect of e-cigarettes. Recent systematic review concluded that e-cigarettes reduced quitting smoking by 28%, but we need more well-designed study to conclude it. Third issue is related to possible gateway effect of e-cigarettes to combustible cigarettes. Several longitudinal studies in the US showed that adolescents with e-cigarette use increased the risk to become combustible cigarette users. Also there are concerns that advertisement and public use of e-cigarettes may renormalize cigarette use. We need to act to fast to reduce negative impact of e-cigarettes even though the evidence is not conclusive now.
